List of required documents for obtaining and renewing a passport in Poland

Persons under the age of 16:

→ Completed application in Ukrainian. You can see an example of such an application here.

→ One 10x15 cm photo. The photo must meet all standards for official documents. Read more about what a photo card should look like.

→ Previous passport (if available). If the child already had a passport, you must submit it for renewal.

→ Birth certificate of the child. If the child was born outside of Ukraine, a translation of the document with an apostille into Ukrainian will be required. A certificate of registration of the child as a citizen of Ukraine will also be required.

→ Passport ID card (if available). If the child already has a Ukrainian passport, a copy of it should be provided.

→ Document proving legal residence in Poland. This may be a residence card, visa or PESEL.

→ Proof of place of residence. As proof of residence, you can provide a rental agreement or a meldunek.

Persons over the age of 16:

→ Completed application in Ukrainian.

→ One 10x15 cm photo.

→ Previous passport (if available).

→ Internal passport (copies of all stamps). Make copies of all stamped pages of your internal passport.

→Identification code.

→ Document proving legal residence in Poland.

→ Proof of place of residence.

 

Obtaining a Ukrainian passport in Poland. Step-by-step guide
